New Patients: What You’ll Need
If you're visiting Eye Rx - Chevy Chase for the first time, completing your new patient forms in advance allows us to review your history and verify your insurance before your appointment. Please have your photo ID, insurance card, and medication list available as you fill out your paperwork.
Returning Patients: Updating Your Information
Even if you’ve seen us before, we may need updates to your forms. Changes in health, medications, address, or insurance can all affect your care. If it has been over a year since your last visit, or if anything important has changed, take a moment to review and update your information prior to your appointment.
If you're unable to complete forms online, have trouble printing them, or aren't sure which ones apply to your visit, we’re here to help. Our Chevy Chase team can email or mail copies, or prepare them for completion in the office during your visit.
